To be considered for a hope, a family must complete a questionnaire that contains detailed medical and family information and sign a release form allowing medical information on the child to be forwarded to Warren’s HOPE, Inc.

Detailed information is compiled by the President on each applicant and presented to the Board at its quarterly board meeting for consideration.

A scoring system is utilized by the Board and is based on the following categories:

• medical necessity of the request
• urgency of the request
• appropriateness of the request (need vs. want)
• benefit to the child

The Board’s decisions are conveyed to the applicant no later than the 15th of the month following the end of the quarter.  All of the payments of hopes will be disbursed directly to the service provider.
